Fillers

Temporary injectable fillers are used for wrinkles, and deep folds such as the nasolabial area (lines that run from nose to mouth) and the glabellar region (frown lines between the brows. They are also used for augmentation of the lips and for filling out acne scars and other types of tissue deficiencies. They are also ideal for "trying on" augmentation before undergoing a more permanent procedure.

Some of the different fillers available:

Artecoll/Artefill:______________________As of today, the only permanent injectable filler is Artecoll. Artecoll has been used for several years. It's now changed it's name to Artefill, It is a mixture of bovine collagen (75%) and tiny plastic PMMA spheres (25%); as the collagen is absorbed, the body surrounds the plastic spheres with its own connective tissue and collagen over 3-4 months resulting in a long-term and possibly permanent fix for wrinkles. Since this filler is permanent, physicians may have some reservations regarding the use of it.
Allergy Test: Yes, since it contains bovine collagen
Longevity: Permanent. (This filler can sometimes go lumpy where it is injected)

Captique:------------------- (technically: non-animal hyaluronic acid) Created by partners, Genzyme and Inamed (formerly McGhan Medical), Captique is based on a non-animal hyaluronic acid. Unmike Inamed's sister [roduct, Hylaform, whichis made from the combs of roosters, Captique is plant based. Hence, no pre-test in necessary is needed as one of it's main components, hyaluronic acid, occurs naturally in the body. Captique is a clear gel that can be injected into the lips as well as wrinkles, to provide a desired augmentation and refreshed, younger appearance.

Once injected it works together with your body's own hyaluronic acid and lasts longer than bovine-derived collagen, or human cadaveric collagen. Captique will be a direct competitor of Q-Med's hyaluronic acid filler, Restylane. Restylane is based on the same type gel which is said to be a highly concentrated, stabilized hyaluronic acid--ideal for wrinkle reduction

Hylaform:_________________ (technically: Hyaluronic viscoelastic gel) Hylaform viscoelastic gel is similar to Rofilan as its main component is hyaluronan but it is made from the combs of roosters. Hyaluronan is necessary to provide moisture and elasticity in the skin. Reportedly, your body's Hyaluronan gradually breaks down over time resulting in drier, wrinkled and aged skin. It has a low allergic reaction rate and is considered safer than most injectable fillers available on the market today.

Typical uses for Hylaform gel:
Wrinkle removal, acne scar, smile lines and frown lines can be filled in to create a smoother appearance and lip augmentation.

Longevity: Hylaform is not permanent, and is eventually absorbed by the body. Hylaform is reported to last up to a year but more typically 4-6 months in most.

Isolagen:____________(technically: your own most vibrant, actively growing cells being reintroduced into your skin) The original US company, Isolagen Inc, was founded in 1995, and Isolagen Europe was formed in London in September 2001. Your surgeon takes a small piece of skin tissue from behind your ear by local anaesthetic at their clinic. Your tissue biopsy is then specially packaged and sent off to the Isolagen laboratory where your collagen-producing cells (fibroblasts) are grown and extracted. After about eight weeks these cells have reached a population of tens of millions of new fibroblasts and are ready to be injected into your skin, Typically, only two further injections are required at two-week intervals.

Longevity: Progressively improve for 18-24 months and should last for several years, 7 years have been reported by the makers.

JuvA©derm:____________ (technically: viscoelastic, transparent hyaluronic acid gel) JuvA©derm is manufactured by L.E.A. Derm which is a subsidiary of the CornA©al group in Paris, France and is packaged in pre-filled syringes containing the hyaluronic gel. JuvA©derm is available in two different concentrations. One ( JuvA©derm 18 ) is for fine lines and wrinkles of 18mg/g, one which is for augmentation of the lips and the newer JuvA©derm 30 for Nasolabials. The JuvA©derm 24 & 30 are 24 mg/g for deeper depressions. JuvA©derm 30, specifically, is better for deeper depressions such as the nasolabials. And of course augmentation!.

Longevity: JuvA©derm is not permanent, it eventually absorbs into the body. JuvA©derm is reported to last typically 4--6 months.

Perlane:___________ (technically: Hyaluronic acid) This product is the same as Restylane only larger in size therefore it takes longer to dissipate. This is usually reserved for deep facial lines such as the nasolabial and glabellar regions. Though it is used for lips as well.

PlasmaGel®:_______________ (technically: patient derived, plasma emulsion with Vitamin C complex) This product lasts about three to five months. After your blood is withdrawn the physician will "spin" the protein out of your blood and mix it with a Vitamin C complex. Your physician then re-injects this mixture resulting in augmentation with your own protein serum. Reportedly, about 7 - 10% of Plasmagel® is permanent.
Allergy Test: not required
Longevity: 3 - 5 months with 7 - 10% possible permanency

Restylane:_______________ (technically: Hyaluronic acid) Restylane was created by Q-Med Laboratories of Sweden. Restylane's make up is based on a non-animal hyaluronic acid. Restylane is injected into the skin or lip wherever augmentation is needed. No pre-test in necessary is needed as one of it's main components, Hyaluronic acid, occurs naturally in the body. Hyaluronic acid is a substance found in all living organisms. If injected within wrinkles, it lessens the appearance of such. Restylane is a clear gel that is injected into the lips as well to provide a desired augmentation and "pouty" lip. Once injected it works together with your body's own hyaluronic acid and lasts longer than bovine-derived collagen. The entire Q-Med hyaluronic line is based on the same type gel from highly concentrated (20mg/ml) stabilized hyaluronic acid - the different "grades", or versions, just vary in particulate size.

Rofilan Hylan Gel:___________________ (technically: viscoelastic, stabilized hyaluronan gel) "Rofilan is a sterile, non-pyrogen, non-animal gel that is cross-linked with a natural acid instead of a chemical compound." Hyaluronan is a substance found in all living organisms and should be injected, by your doctor, intradermally with a 30 gauge needle. It is not permanent but does last longer than collagen or other forms of hyaluronan. The absorption time of hyaluronan depends upon the length of the molecules. The hyaluronan molecules in Rofilan Hylan Gel is 2.5 million Dalton, whereas other hyaluronan injectables are usually about 1.5 million Dalton). Hence a longer augmentation result is theorized.

Zyderm & Zyplast:_____________________There are 3 forms of injectable Collagen, Zyderm I, Zyderm II and Zyplast. Composed of bovine collagen, the material is mixed with sterile saline (salt water) and a local anesthetic called lidocaine. All 3 require skin testing 4 weeks prior to use. A small amount of collagen is injected into the soft portion of the forearm, about 3 finger widths from the bend of the elbow. This helps standardize the area of injection and makes sure everyone knows where to watch for a reaction. Reactions can include redness, itching or puffiness, similar to a TB test. Three percent of tested patients experience a positive reaction. These patients are not able to use injectable collagen and may find that other injectable agents or procedures are available to soften their wrinkle lines. A small number of those who test negative may experience a later reaction (1-2%) to the actual therapeutic product.

Zyderm I is appropriate for very fine lines around the eyes and around the mouth (periocular and perioral, respectively). Product is injected with a very fine needle to reduce damage at these fragile sites. Also, due to the structure and concentration of this product, your physician will overcorrect treated areas 200%. This means you will have red and/or puffy areas for a few days until surrounding tissue absorbs the saline.

Zyderm II is a more concentrated form of bovine collagen. It is more appropriate for deeper wrinkle lines such as those of the globella (the space between the eyebrows) and the naso-labial folds (lines that extend from the nose to the edge of the mouth). Your physician will only have to overcorrect the treated area approximately 150%. Again, within a few days the redness and puffiness should resolve.

Zyplast is a more stable product formed by incorporating glutaraldehyde with the bovine collagen. Although it is only 35mg/ml concentration, the stability provided by the chemical process makes it an ideal treatment for deeper tissue defects such as the naso-labial lines and oral commissures (the creases at the edges of the mouth). Zyplast is NOT to be injected into the globellar lines (area between the eyebrows) due to the risk of clotting off blood vessels in this area. There have been several reports of this type of complication, so make sure that those frown lines are treated with other options, including Zyderm II or Botox. Over-correction with Zyplast is not recommended. Zyderm I or Zyderm II may be layered more superficially after Zyplast is injected deeply into the dermis in certain more accentuated furrows and folds.

Everyone wants to know how much it is going to hurt. Yes, it will hurt, but as the product contains a local anesthetic, it goes numb quickly. The numbness lasts for a few hours. Each area that is treated will receive several serial injections along the length of the wrinkle line. The only exception is the vermillion border of the lips where a single long injection is done along the length of the lip. The doctor typically massages each treated area after injection to disperse the collagen evenly.

Pain
It's reall hard to say if you will find injections with fillers very painfull You can ask for local anesthetic, regional "nerve block" or Emla cream for pain relief.
Doctors will charge you more for local anesthetic and nerve blocks, but Emla cream is free usually.
